La definizione ufficiale di pressure injury (PI) è stata aggiornata nel 2016 dal National Pressure Injury Advisory Panel (NPIAP), formerly National Pressure Ulcer Advisory Panel (NPUAP), e successivamente incorporata nelle linee guida internazionali del 2019 (Hess, 2020; Gefen et al., 2021).

Il termine "injury" sostituisce "ulcer" per includere anche lesioni con cute intatta (Stage 1 e Deep Tissue Pressure Injury), dove un'ulcera non può esistere senza un'lesione, ma un'lesione può esistere senza ulcera (Edsberg et al., 2016).

Definizione NPIAP del 2016

La definizione aggiornata recita: «A pressure injury is localized damage to the skin and underlying soft tissue usually over a bony prominence or related to a medical or other device. The injury can present as intact skin or an open ulcer and may be painful. The injury occurs as a result of intense and/or prolonged pressure or pressure in combination with shear. The tolerance of soft tissue for pressure and shear may also be affected by microclimate, nutrition, perfusion, co-morbidities and condition of the soft tissue» (Hess, 2020; Hanson et al., 2023).

Questa definizione è stata adottata nelle linee guida cliniche internazionali (Clinical Practice Guideline, CPG) del 2019, che descrivono la PI come «localised damage to the skin and/or underlying tissue, as a result of pressure or pressure in combination with shear; PUs/PIs usually occur over a bony prominence but may also be related to a medical device or other object» (Gefen et al., 2021; Liu et al., 2024; Pan et al., 2023).

Negli Stati Uniti, Canada, Australia e parte del Sud-Est asiatico il termine "pressure injury" è stato ampiamente adottato, mentre in Europa (EPUAP) permane il termine "pressure ulcer" (Gefen et al., 2021; Velozo et al., 2025). Gli eventi eziologici e i fattori scatenanti sono però identici e tutti i termini si riferiscono allo stesso fenomeno (Gefen et al., 2021).

Elementi chiave della definizione

  • Pressione e/o shear: cause meccaniche primarie da carico prolungato o intenso (Gefen et al., 2021; Kandi et al., 2022)- Sede tipica: prominenze ossee o device medici (Edsberg et al., 2016; Velozo et al., 2025)- Fattori di tolleranza tissutale: microclima, nutrizione, perfusione, comorbidità (Gefen et al., 2021; Pan et al., 2023)- Presentazione clinica: cute intatta o ulcera aperta, con possibile dolore (Gefen et al., 2021; Velozo et al., 2025)La revisione del 2016 ha incorporato la device-related PI, riconoscendo che i dispositivi medici e non medici costituiscono una delle eziologie (Edsberg et al., 2016; Velozo et al., 2025). Il cambiamento terminologico da "ulcer" a "injury" riflette le evidenze istopatologiche che mostrano come danni tissutali precoci avvengano prima della visibilità clinica (Edsberg et al., 2016; Gefen et al., 2021). La necessità di una definizione unificata che integri concetti di skin failure e inevitabilità rimane dibattuta (Levine, 2026).
